The Elms Medical Practice
The Elms Medical Practice has provided healthcare to the residents of the Hoo Peninsula since the inception of the NHS.
We have a clear focus on quality and safety, as well as evidence-based care.
The practice participates in education of medical students from Kent and Medway Medical School, and hosts clinicians for postgraduate supervision, including Foundation Year 2 doctors and General Practice trainees.

Services are delivered from our main site in Hoo St Werburgh, complemented by branches in Grain, High Halstow and Allhallows.
The pandemic and the initiative to set up Primary Care Networks has accelerated deployment of new professionals in primary care, as well as adoption of a variety of technologies. The doctors are supported by professionals including Clinical Pharmacists, Nurse Practitioners, Paramedics, First Contact Physiotherapists, Physicians Associates and Social Prescribing Link Workers.
In collaboration with Highparks, we delivered the Covid-19 immunisation programme to the locality.
Patients residing more than 1.6 km from a community pharmacist may use the dispensing service available at the practice.
We welcome new patients. Please collect registration forms from the practice, or alternatively download them from our website.
