Mental Health Practitioner (MHP)
An MHP can assess, diagnose and treat the mental health needs of patients. They work with a case load of new patients and those requiring ongoing follow up care, referring patients where necessary to the correct external organisations.
We have one MHNP within Medway Peninsula PCN.
